We encourage you to use the provider tools at the right for your dental or medical patients. We aim to make your job easier so don't hesitate to let us know if there's anything else we can assist with.
LifeMap offers a short term medical product that provides temporary health coverage for members who are typically between jobs or waiting for new coverage to kick in. As such, we do not contract with a network of medical providers for benefits. The plan does not cover pre-existing conditions so no pre-authorizations are needed. As a LifeMap contracted dental provider, we can provide you with a pool of potential patients, predictable reimbursement amounts and direct payment. We are continually seeking dental providers to join our growing network to provide the best access to care for our members.
The LifeMap Dental Network contracts with providers in several states. To join our network, simply contact the following office and let them know you’d like to apply:

We accept both paper and electronic claims. Find out how to submit a claim to LifeMap.
Receiving claims electronically is our preferred method to ensure ease of doing business with us. Make sure you are registered with Availity or the Dental Center to submit eClaims.
Claims payments can be made to all providers using Electronic Funds Transfer (EFT), except providers outside our service area.
To register for EFT complete the EFT Enrollment Form and submit to LifeMap.
