Discontinuing Individual Dental New Policy Purchases

LifeMap will no longer sell Individual Dental products for effective dates of Oct. 1, 2023, and beyond. This change applies to the following Individual Dental plans:

• Individual Incentive
• Dollar Based Dental
• Individual Exclusive Provider Organization Dental (Willamette)
• Dental Vision rider

These products will no longer be available for purchase through the LifeMapco.com website after Sept. 14, 2023. The last policy effective date for purchase will be Sept. 15, 2023.

Important dates related to this change:

• Last day to purchase |  Sept. 14, 2023
• Application postmark date*  |  Sept. 14, 2023
• Last effective date  |  Sept. 15, 2023

There is no impact to our current active policies. Existing and new customers will continue to receive excellent customer care from our servicing teams.
