Moving Off an Employer's Plan

If your LifeMap coverage through your employer is ending, you may be able to continue your insurance through Portability, Conversion or a Direct Bill option. With one of these options you don’t need to complete a new Evidence of Insurability form, answer any health questions or undergo a medical exam to continue coverage. 

What is Portability?

Portability means continuing coverage under the group policy when your coverage ends do to a loss of eligibility.  Premium for coverage continued under a Portability provision is billed to you directly from LifeMap.  Portability is a smart choice because you are able to keep your coverage at the same great rates you are currently paying.*

To qualify for Portability, you must be terminating coverage for reasons other than:

  1. your disability; 
  2. a military leave that extends beyond the period provided under the policy; or
  3. your retirement. 

Please note: There may be other limitations or restrictions regarding eligibility, so please consult your Certificate of Coverage before applying for this option.

LifeMap Coverage that you may be able to Port:

  1. Basic Life
  2. Voluntary Life
  3. Dependent Life
  4. Voluntary Short Term Disability
  5. Voluntary Long Term Disability

What is Conversion?

Conversion means continuing your life insurance under an individual life insurance policy when your coverage under the group policy ends.  Conversion to an individual life insurance policy is a smart choice because the individual policy is not tied to the group policy. This means that your coverage does not end even if the group policy terminates.

To qualify for Conversion, your coverage may be terminating for any reason.

LifeMap Coverage that you may be able to Convert: 

  • Basic Life
  • Voluntary Life
  • Dependent Life

What is Direct Bill?

Direct Bill means continuing the same coverage you have under the group policy, but your coverage is no longer tied to the group. Premium for coverage continued under a Direct Bill provision is billed to you directly from LifeMap. 

To qualify for Direct Bill, your coverage may be terminating for any reason.

LifeMap Coverage that you may be able to continue with Direct Bill:

  • Critical Illness and Cancer Care
  • Accident Only 

Important to Know
It’s up to your employer to notify you of your options if your coverage is ending for any reason; however, you can also speak with your HR manager to learn more about your eligibility to continue coverage and the provisions specific to your company’s policy. 

You will have 31 days from the date your coverage under the group policy ends (or the date the group’s policy terminates) to apply for Portability, Conversion or Direct Bill. Portability is not available if you opt for coverage under a Conversion Policy.
